Welcome to our private duty program, Partners In Community Health (PCH)! PCH is a nurse owned and operated business. We work closely with our affiliated Medicare certified home care agency All About You Collaborative Health Care Services, LLC. Our Mission is to assist you in reaching your health goals at home with the vision of planning ahead and not in crisis. Our care team will work with you to coordinate community services so you can have peace of mind and remain safely in your home. A comprehensive and individualized plan will be developed with you to ensure you have the support you need. Our private duty staff undergo screening at time of employment including background checks.They are bonded and covered under the program’s insurance. Our staff are trained to respect your privacy, property and wishes while in your home. In addition, they are trained to recognize change in health condition.
